Selektive Aufmerksamkeit bei Alkoholabhängigen in der medizinischen Rehabilitation. Konzept, Prävalenz, Remission.

  • Es wird eine Personenbezogene (differenzielle) Kontinuitätshypothese formuliert, in der angenommen wird, dass Personen, die kognitive Defizite als Folge massiven Alkoholkonsums erleiden, drei aufeinanderfolgende Schädigungsstufen durchlaufen. Ziel der vorliegenden Untersuchung war es, eine Prävalenzrate von Störungen der (selektiven) Aufmerksamkeit und der nonverbalen Intelligenzfunktionen als Maße für exekutive Funktionsstörungen und der Gedächtnisleistungen bei alkoholabhängigen Patienten zu Beginn einer medizinischen Rehamaßnahme zu ermitteln. Zudem wurde zu einem zweiten Messzeitpunkt eine Remissionsrate festgestellt. Im Zeitraum von November 2005 bis Ende April 2006 wurden in den Kliniken Wied 87 konsekutiv aufgenommene Patienten mit einer neuropsychologischen Testbatterie untersucht. Es zeigte sich eine Prävalenzrate von 49.4% für Beeinträchtigungen nicht sprachlicher Intelligenzfunktionen und von 43.7% für Störungen der selektiven Aufmerksamkeit. Gedächtnisdefizite traten lediglich bei 15% auf. Nach fünf Wochen fanden sich deutliche Verbesserungen der selektiven Aufmerksamkeit, während sich die Gedächtnisleistungen kaum verbesserten.
  • At least since the first description of the alcoholic Korsakoff- syndrome in the second half of the 19th century, cognitive deficits as a result of massive alcohol consumption are known. A linear correlation between consume related variables (i.e. the amount of alcohol consumed in lifetime) and the decline in cognitive functions (i.e. memory impairments) had not yet been found as would be expected by the in 1971 by Ryback formulated continuum theory. Nevertheless quantitative and qualitative differences in the cognitive abilities of alcoholic patients with and without Korsakoff- syndrome do exist. On base of these observations an individual-related continuum hypothesis is formulated. In the stage model of the individual-related continuum hypothesis it is postulated that individuals who develop cognitive impairments in consequence of long-term heavy alcohol consumption pass two different stages before developing a alcoholic Korsakoff- syndrome. In this study the prevalence of impairments in the executive functions "focused attention" and "nonverbal reasoning" and memory impairments should be found. Between November 2005 and April 2006 87 alcohol dependent inpatients of the "Kliniken Wied" (an inpatient rehabilitation center for alcohol and drug dependence) without any previous identified brain damage, without any medication influencing the central nervous system and without any other F1x.x-diagnoses (except F17.x) or F2x.x-diagnosis were tested with a neuropsychological test battery. A prevalence of impairments in the ability of nonverbal reasoning of 49.4% could be detected. 43.7% of the patients had impairments in their focused attention. Only 15% showed impairments in their memory functions at the beginning of their rehabilitation program. Five weeks after the first examination most patients improved their performance in the test of focused attention whereas the memory impairments in most of the cases did not recover. The results support the expectations of the individual-related continuum hypothesis about the recovery of cognitive impairments in consequence of long-term heavy alcohol consumption.
