Einfluss von Ausdauertraining auf Patienten mit Angst- und somatoformen Störungen sowie mit kardiovaskulaerer Dysregulation

Influence of aerobic exercise on patients with anxiety- and somatization disorders as well as with autonomic cardiovascular dysregulation

  • Anhand einer randomisierten kontrollierten Stichprobe von stationären Patienten mit Angststörungen (ICD 10: F40, F41 und somatoformen Störungen (ICD10:F45) wurde der Einfluss eines systematischen aeroben Trainingsprogramms auf psychologische Parameter, sowie Parameter der autonomen kardiovaskulären Regulation (Barorezeptorensensitivität, Herzratenvariabilität und Reaktivität von HRV und BRS) untersucht. Anhand der Studie sollten folgende Fragen beantwortet werden: 1.) Wie sind die psychologischen Auswirkungen des Ausdauertrainings? Die bekannten positiven Einflüsse von Ausdauertraining auf psychische Symptome, wie z. B. Angst und Depression wurden grösstenteils anhand von Studien mit nichtklinischen Probanden gewonnen. Ziel unserer Studie war die Überprüfung dieser Effekte in einer klinischen Stichprobe mit Patienten. 2.) Führt ein systematisches aerobes Ausdauertraining bei Patienten, die zusätzlich zu ihrer psychischen Störung eine Dysfunktion der autonomen Regulation aufweisen zu einer Normalisierung dieser Regulationsstörung. 3.) Kommt es bei einem körperlichen Training zu einer Verminderung der Reaktivität von Kennwerten der autonomen kardiovaskulären Regulation beim Auftreten psychosozialer bzw. emotionaler Stressoren? 89 Patienten mit vollständigem Datensatz wurden zur Auswertung herangezogen. Die Ergebnisse der Studie zeigen dass ein kontinuierliches Ausdauertraining zu einer deutlichen Reduktion der psychologischen Symptomatik bei Patienten mit Angststörungen und somatoformen Störungen führt. Auf der Ebene der physiologischen Effekte bestätigen die Ergebnisse unsere Hypothese, dass Ausdauertraining die Herzratenvariabilität, Barorezeptorensensitivität und die Reaktivität dieser Parameter verbessert, wobei diese Veränderungen auf Patienten mit unbeeinträchtigter kardiologischer Regulationslage beschränkt war.
  • On the basis of a randomized controlled sample of stationary patients with anxiety disorders (ICD 10: F40, F41) and somatization disorders (ICD10: F45), we examined the influence of a systematic aerobic exercise training program on psychological parameters, as well as on parameters of the autonomous cardiovascular regulation (baroreflex sensitivity, heart rate variability and reactivity of HRV and BRS). The study should provide an answer to the following questions: 1.) What are the psychological effects of aerobic exercise? The well-known positive influences of aerobic exercise on psychological symptoms, e.g. anxiety disorders and depression were so far mostly conducted on the basis of studies with not-clinical patients. The aim of our study was to examine these effects in a clinical sample with in- patients. 2.) Does systematic aerobic exercise- training with patients, especially with patients who, in addition to psychological disorders, show a dysfunction of the autonomic cardiovascular regulation system lead to normalization of this dysregulation? 3.) Does regular aerobic exercise training lead to a reduction of the reactivity of characteristic values of the autonomic cardiovascular regulation with the occurrence of psychosocial and/or emotional stress? 89 patients with complete data record were recruited for the evaluation. The results of the study showed that systematic aerobic exercise leads to a clear reduction of the psychological symptoms in the group of patients with anxiety and somatization disorders. On the level of physiological effects the results confirm our hypothesis that systematic aerobic exercise training improves the heart rate variability, baroreflex sensitivity, and the reactivity of these parameters, whereby these changes are limited to patients with normal autonomic cardiovascular regulation.
