Gesundheitsrelevantes Risikoverhalten und subjektive Gesundheit im Nachwuchsleistungssport. Eine Analyse jugendlicher Leistungssportler aller olympischen Sportarten im Vergleich mit einer gleichaltrigen Referenzstichprobe

Health related risk behavior and subjective health in elite adolescent athletes

  • Im Leistungssport ist ein gesunder Körper die Hauptvoraussetzung für das Erbringen sportlicher Höchstleistungen. Allerdings müssen Leistungssportler tagtäglich in Training und Wettkampf gesundheitliche Risiken eingehen. Aus diesem Dilemma wird ein Trilemma, sobald es sich um jugendliche Leistungssportler handelt, da das Jugendalter mit all den Veränderungen auf psychischer, physischer und sozialer Ebene, die es mit sich bringt, häufig das Einstiegsalter für gesundheitsrelevante Risikoverhaltensweisen wie den Konsum von Alkohol, Tabak und Marihuana bildet. Diese können zusätzlich Auswirkungen auf die Gesundheit und damit das Erbringen sportlicher Leistung haben. Aus diesem Grund war es das Ziel dieser Dissertation, das gesundheitsrelevante Risikoverhalten und die subjektive Gesundheit in der speziellen Gruppe jugendlicher Leistungssportler zu untersuchen, mit einer jugendlicher Referenzstichprobe bestehend aus Nicht-Leistungssportlern zu vergleichen und assoziierte Determinanten - sowohl individueller als auch kontextueller Natur - zu identifizieren. Dazu wurden im Rahmen der GOAL Study (German Young Olympic Athletes" Lifestyle and Health Management Study) 1.138 jugendliche Leistungssportler aller olympischer Disziplinen schriftlich befragt. Ausgewertet wurden neben der subjektiven Gesundheit der Konsum von Alkohol, Tabak und Marihuana, das Ernährungsverhalten und die Nutzung von Nahrungsergänzungsmitteln. Als Vergleichspopulation wurde der KiGGS (Kinder- und Jugendgesundheitssurvey) des Robert Koch-Instituts herangezogen. Es zeigte sich, dass jugendliche Leistungssportler zwar selten rauchten (3%) oder Marihuana konsumierten (3%), aber dass sie durchaus Alkohol zu sich nahmen (86%) und insbesondere Binge Drinking betrieben (24%). Bezüglich des Rauchverhaltens, des Konsums von Marihuana und des Jemalskonsums von Alkohol zeigten sie jedoch niedrigere Prävalenzen als jugendliche Nicht-Leistungssportler. Bei der Ernährung der Athleten ergaben sich teilweise Defizite. Lediglich 52% der Athleten nahmen täglich eine Auswahl an vitamin- und ballaststoffreichen Nahrungsmitteln zu sich, jedoch waren dies mehr als in der Referenzpopulation. Neun von zehn Athleten nutzten Nahrungsergänzungsmittel, davon konsumierten rund 30% täglich Supplemente. Die Mehrheit der Athleten bezeichnete ihre Gesundheit als gut oder sehr gut (89%), was einem größeren Anteil entspricht als in der Referenzgruppe (86%). Es fanden sich eine Vielzahl an individuellen und kontextuellen Determinanten mit den einzelnen Risikoverhaltensweisen und der subjektiven Gesundheit verknüpft. Insgesamt lässt sich festhalten, dass Beratung und Aufklärung bezüglich der einzelnen Risikoverhaltensweisen immens wichtig erscheinen. Beispiele für bestehenden Aufklärungsbedarf sind der übermäßige Alkoholkonsum, welcher Auswirkungen auf die Leistungsfähigkeit haben kann, die Einnahme von Nahrungsergänzungsmitteln wie Kreatin, welche für Jugendliche nicht unbedenklich ist, und der häufig vorliegende nicht-tägliche Konsum von vitamin- und ballaststoffreichen Lebensmitteln, welcher wichtig für eine adäquate Ernährung ist. Daher sollten bereits Athleten in niedrigeren Kaderstufen Experten (z. B. Ernährungsberater) zur Seite gestellt werden. Aber nicht nur die Athleten sollten besser geschult werden sondern auch deren Trainer, Betreuer und Eltern, die allesamt wichtige Bezugspersonen darstellen.
  • In the context of competitive sports a healthy body is the most important requirement to accomplish top performances. However, elite athletes are faced with serious health risks on a daily basis due to long training sessions and frequent participation in competitions. This represents a real dilemma which even becomes a trilemma for adolescent elite athletes as the period of adolescence " entailing a number of changes on psychological, physical and social level " often is the onset age for risky health-related behaviors such as the consumption of alcohol, tobacco and marijuana. This can have substantial effects on the health and consequently on the accomplishment of sports performances. For this reason, the present dissertation aimed at investigating health-related risk behaviors and the individual health in the specific group of adolescent elite athletes, to compare the results with a reference sample of adolescent non-elite athletes and to identify associated determinants of both individual and contextual nature. In order to do this, 1,138 adolescent elite athletes of all Olympic disciplines were questioned by letter in the framework of the GOAL Study (German Young Olympic Athletes" Lifestyle and Health Management Study). In addition to the individual health, the consumption of alcohol, tobacco and marijuana, the nutritional behavior and the use of dietary supplements were evaluated. For comparison, the data were combined with data from the German Health Interview and Examination Survey for Children and Adolescents (KiGGS) from the Robert Koch-Institute. It could be shown that adolescent elite athletes indeed rarely smoke (3%) or consume marijuana (3%) but that they do drink alcohol (86%) and are especially engaged in binge drinking (24%). However, concerning smoking habits, the consumption of marijuana and the ever-consumption of alcohol, lower prevalences were revealed compared to those of adolescent non-elite athletes. In regard to the nutrition of athletes, deficits were identified in parts. Only 52% of the athletes were consuming a range of foods rich in vitamin and fiber on a daily basis. However, this represented an elevated number compared to that of the reference population. Nine out of ten athletes were taking dietary supplements, out of which 30 % were consuming supplements on a daily basis. The majority of the athletes considered their health as good or very good (89 %) which represents a higher percentage as that of the reference group (86 %). A variety of individual and contextual determinants was found to be associated with health-related risk behaviors and the individual health status. A summarizing conclusion is that thorough counseling and informing about risk behaviors seem to be extremely important. Examples that prove the existing need for information are the excessive consumption of alcohol, which can have serious effects on the capacity to perform outstandingly, the intake of dietary supplements such as creatine, which is critical for adolescents, and the fact that very often adolescent elite athletes do not consume foods rich in vitamin and fiber on a daily basis, which would be important for an adequate nutrition. This is why experts (e.g. nutrition counselors) should be available for athletes as early as from when they are at lower squad-levels. But it is not only the athletes themselves that should get a better education about these issues. Also the coaches, advisors and parents, who all represent important persons of reference, should profit from improved assistance.
